What the f*ck is happening to me? (and why your body feels different in your 40s)

This is the question I hear most often when women first start talking to me about what’s changed in their body in their late 30s and 40s. Not always spoken exactly like that, but it’s usually there underneath everything else they are trying to explain. A sense that something feels different, and no matter how much effort they put in, they can’t quite understand why.


Most women don’t experience this as one clear moment. It’s gradual. Sleep starts to shift slightly, not always bad, but different. Lighter, more broken, or more inconsistent than it used to be. Many women describe waking earlier than normal without trying to, or lying awake at night with a mind that feels like it’s switched on before they are ready for it.


During the day, things feel different too. Energy is less predictable. Appetite feels less stable. Cravings show up at times they didn’t used to, especially later in the day when they finally sit down. Nothing looks dramatically different on paper, but the experience of their body starts to feel unfamiliar.


Then body composition starts to shift. Clothes fit differently in places they never used to. Fat distribution changes slightly. Training doesn’t seem to “hold” results in the same way it used to.


And this is usually where the question shows up: What the f*ck is happening to me?


Because from the outside, nothing obvious has changed. Most women are still doing the same things. Still training. Still trying to eat well. Still being consistent. So the assumption becomes that something must have broken, usually metabolism, or hormones, or just “age catching up”.


But when we look at the actual evidence, that explanation doesn’t hold on its own.



Metabolism is not “slowing down”


One of the most persistent beliefs is that metabolism slows dramatically in midlife and that this is what drives weight gain during the menopause transition.


But large-scale metabolic research does not support this.


A 2021 study examining energy expenditure across the lifespan (1) found that resting metabolic rate remains remarkably stable from early adulthood into midlife when adjusted for fat-free mass. In simple terms, metabolism is not steadily declining in the way most women are told it is.


So if metabolism itself is not significantly slowing down, the question becomes: what is actually changing?


The answer is that multiple systems shift at the same time, and it is the interaction between them that changes how the body looks and feels.



It is not one change. It is a system shift.


In perimenopause and menopause transition, ovarian hormone production begins to fluctuate and eventually decline. This does not just affect reproductive function. Estrogen in particular has widespread effects across the brain, muscle tissue, liver, fat tissue, and the nervous system.


A 2024 review on estrogen deficiency (2) highlights that estrogen receptors are present throughout the body and play a role in energy regulation, vascular function, neurological signalling, and metabolic balance. When estrogen signalling becomes less consistent during perimenopause, multiple systems begin to respond differently to the same inputs.


This is where things start to feel “off” in real life.


Not because one thing has broken, but because multiple small regulatory systems are shifting at the same time.



Insulin sensitivity, Postgrandial Glucose & Energy Regulation


One of the key changes seen during the menopausal transition is how the body responds to insulin and glucose.


A 2021 review (3) found that estrogen plays a key role in maintaining insulin sensitivity in skeletal muscle, the liver, and adipose tissue. As estrogen begins to fluctuate and decline during perimenopause, this protective effect becomes less consistent, meaning some women become less sensitive to insulin and less efficient at regulating blood glucose.


Research also suggests these changes can begin earlier than many women realise. A cohort study in midlife women (4) found that changes in glucose regulation and insulin sensitivity can occur during the early menopausal transition, even before significant changes in body weight are present. The researchers also observed higher postprandial (after-meal) glucose concentrations as women progressed through the menopausal transition, indicating that blood glucose remained elevated for longer after eating than it had previously.


This matters because insulin sensitivity is about far more than blood sugar. It influences how efficiently energy is used, how stable appetite feels, how long you stay satisfied after meals, and how predictable your body's response to food becomes.


When this system becomes less efficient, the lived experience is often subtle at first. You might notice your energy crashing after meals, feeling hungry again sooner than expected, or experiencing stronger cravings throughout the day. Combined with changes in sleep, body fat distribution, and recovery, these alterations in glucose regulation can contribute to the gradual body composition changes many women notice during perimenopause and menopause.




Body composition shifts are not random


Another consistent finding in the literature is a change in body composition distribution during the menopausal transition.


A 2022 review (5) found that women often experience an increase in fat mass and a shift toward central fat storage during midlife, even when total weight change is modest.


This is not simply a “calorie imbalance story”. It reflects the interaction between hormonal changes, energy regulation, sleep disruption, and behavioural adaptation over time.


Estrogen plays a role in how fat is distributed and how energy balance is regulated across tissues (6). When estrogen signalling declines, fat storage patterns tend to shift, particularly toward abdominal regions in some women.


So what women see in the mirror is not just “weight gain”. It is a redistribution of where and how the body stores energy.



Sleep changes are a major driver most women underestimate


Sleep is one of the most underestimated parts of this entire picture.


A 2024 clinical review (7) found that sleep disturbance becomes significantly more common during the menopausal transition, including increased night waking, reduced sleep quality, and more fragmented sleep architecture.


This matters because sleep is not separate from body composition. It directly influences appetite regulation, cravings, stress tolerance, recovery, and training output.


When sleep becomes more fragmented, appetite regulation becomes less stable. Cravings increase. Recovery slows. Training tolerance decreases. And all of that feeds back into how the body responds to food and exercise.


This is one of the reasons women often describe doing everything the same, but getting a completely different result.


Because sleep alone can change the outcome of the same behaviours.



Why weight gain feels like it “comes out of nowhere”


When you start to put all of this together, the experience begins to make more sense. It is not that metabolism suddenly drops in isolation and causes weight gain on its own. It is that multiple systems begin shifting at the same time, and the interaction between them changes how the body responds to the exact same inputs it used to handle without issue.


Sleep becomes less stable, insulin sensitivity becomes less consistent, fat distribution patterns shift, recovery from training changes, and overall energy regulation becomes less predictable. None of these changes on their own fully explains what women experience, but together they alter the way the body responds to training, nutrition, stress, and recovery in a way that feels unfamiliar.


From a lived perspective, this is where the confusion usually starts. Most women will say some version of “I haven’t changed anything, but my body has,” and that experience is consistent across the women I work with.



The part most women get stuck in


This is usually where the cycle begins. The natural response is to assume that more effort is needed in the same direction that used to work. So food gets reduced slightly, training increases, and there is a push to be more disciplined and more controlled with everything.


Sometimes there is a short-term response, but because the underlying system has changed, that approach rarely produces stable results over time. The outcome fades, frustration builds again, and the cycle repeats itself: try harder, see a short result, lose it, restart, and repeat.


Over time, this shifts from being about the body itself to something much more psychological. It becomes about confidence, trust, and predictability, because nothing feels reliable anymore. The same actions no longer guarantee the same outcomes, and that creates a constant sense of uncertainty.



The real issue is mismatch, not failure


In my experience coaching women through this stage, the issue is rarely a lack of discipline or consistency. Most women are still trying very hard. The problem is mismatch. A stable, fixed approach being applied to a body that is no longer responding in a stable way.


Once that is understood, everything changes. It stops being personal, it stops being about failure, and it stops being about doing things “wrong.” Instead, it becomes a systems problem that can actually be worked with in a structured way rather than guessed at.
